OPSITE FILM DRESSING

A thin transparent film dressing protects a wound or skin site from water and external contamination, allows the wound to be observed without removal, and conforms to body contours to maintain a consistent seal during daily activity. Opsite Film Dressing is a transparent waterproof polyurethane film dressing from Smith & Nephew. Moisture vapour permeable. Self-adhesive. Three sizes: 6cm x 7cm, 12cm x 12cm, and 15cm x 20cm. Opsite Film Dressing is a regulated medical device from Smith & Nephew and part of the Opsite range of film dressings. It is a transparent polyurethane film coated with a gentle acrylic adhesive on its underside. The film adheres directly to the periwound or surrounding skin and forms a conformable waterproof cover over the wound or skin site. There is no central absorbent pad, no wound contact material distinct from the film itself, and no secondary component. The film is the entire dressing.

Opsite Film Dressing is the plain film format within the Opsite range. Other formats in the Smith & Nephew Opsite family include Opsite Flexifix, which is supplied as a continuous roll for cutting to size, and Opsite Post-Op, which combines a film outer layer with an absorbent wound contact pad. Opsite Film Dressing in discrete pre-cut sizes is the straightforward choice for applications where a standard film dressing in a fixed size is needed.

The polyurethane film has two properties that work alongside each other. It is waterproof: liquid water cannot penetrate the film from outside when it is intact and the edges are adhering to the surrounding skin. It is also moisture vapour permeable: water vapour produced by the skin beneath can pass through the film structure and evaporate from the outer surface. The combination provides a waterproof barrier to external water while allowing the skin to breathe beneath the dressing.

The transparent film allows the wound or skin site to be observed at any time without removing the dressing. This is particularly useful for monitoring IV cannula insertion sites for early signs of inflammation, checking wound appearance during healing, and assessing skin integrity under the film without the disruption of a change.

Opsite Film Dressing has no absorbent capacity and must not be applied to exuding wounds. Exudate pooling under the film will macerate the wound and surrounding skin and lift the film edges. Apply only to dry or minimally producing wound surfaces. Apply to completely dry periwound skin. Apply without tension to avoid skin stripping at the film edges.

Waterproof Film Barrier

The polyurethane film is impermeable to liquid water. When applied with the edges adhering fully to the surrounding skin, external water from showering or bathing cannot reach the wound through the intact film. This allows patients to wash normally without covering the wound, supporting hygiene and patient comfort during recovery. The waterproof seal is only effective while the film is intact and all edges are adhering. Any lifted edge compromises the seal and requires the dressing to be replaced.

Moisture Vapour Permeability

The polyurethane structure of the film allows water vapour molecules to pass through. Water vapour from normal skin transpiration and wound surface activity passes outward through the film and evaporates from the outer surface. This prevents the accumulation of moisture beneath the film that would occur under a fully impermeable covering, reducing the risk of maceration of the wound surface and periwound skin during extended wear. The film breathes while remaining impermeable to liquid water.

Wound Visibility

The transparent film allows clear observation of the wound, cannula site, or skin surface beneath without removing the dressing. A clinician or patient can assess wound healing progress, check for early signs of inflammation or infection, observe changes in wound appearance, or confirm that a cannula site remains clean and undisturbed at any time without the disruption of a dressing change. This reduces unnecessary removals and minimises interference with the wound environment.

Film Conformability

The thin, flexible polyurethane film conforms to body contours and moves with the skin. This conformability maintains a consistent edge seal on curved surfaces, over joints, and in anatomically awkward locations where a rigid dressing would create lift points. The film is flexible enough to allow normal joint movement without the dressing edges peeling from the skin during activity.

Preparation

• Wash hands and use gloves.
• Ensure the wound surface is dry and not producing exudate.
• Dry the periwound skin completely.
• Do not apply creams, lotions, or sprays to the skin that will receive the film.

Application

• Select the appropriate size: 6cm x 7cm, 12cm x 12cm, or 15cm x 20cm.
• Remove the release liner.
• Apply the film centrally over the wound or skin site.
• Apply without tension. Do not stretch the film during application.
• Smooth from the centre outward to ensure full skin contact and remove air pockets.

Removal

• Lift one corner of the film and peel slowly back along the skin surface.
• Do not pull upward at a sharp angle.
• Support the skin ahead of the peeling edge.
• On fragile or elderly skin, dampen the film edge briefly before removal to reduce adhesive force.

Never apply Opsite Film Dressing under stretch. Film applied under tension will recoil against the skin and can cause blistering or skin stripping, particularly on elderly, fragile, or steroid thinned skin. Apply relaxed and smooth from the centre outward. The same applies at removal: peel slowly along the surface, never upward at an angle.

Opsite Film Dressing 6cm x 7cm

A small film dressing for compact wound sites, digit wounds, small incision cover, and IV cannula fixation sites where a smaller film provides adequate coverage with a secure adhesive border on all sides.

Opsite Film Dressing 12cm x 12cm

A medium square film for moderate-sized wound surfaces, post-operative closed wound cover, and sites where the 6cm x 7cm does not provide adequate coverage or where a square format suits the wound geometry.

Opsite Film Dressing 15cm x 20cm

A large film for more extensive wound areas, longer surgical incision cover, and sites requiring broad transparent film protection. Also suitable as a secondary film overlay over primary wound contact materials, where a transparent outer film is the chosen secondary dressing. Under clinical supervision for wounds at this scale.

All three sizes use the same polyurethane film material with the same waterproof and moisture vapour permeable properties.

When Not to Use Opsite Film Dressing

• Wounds producing exudate. No absorbency means exudate pools beneath the film and causes maceration.
• Infected wounds.
• Wet, greasy, or compromised periwound skin where the film adhesive cannot bond.
• Fragile or steroid thinned skin where film application and removal carries a high risk of skin trauma, without clinical guidance.
• Known sensitivity to polyurethane or acrylic adhesive components.

Precautions

• Apply to completely dry intact skin only.
• Apply without tension.
• Remove slowly along the skin surface, not at a sharp upward angle.
• Support the skin at the peeling edge, particularly on fragile skin.
• Single use only.

Adverse Effects

• Skin blistering or stripping if applied under tension or removed sharply from fragile skin.
• Wound or skin maceration if applied over an exuding wound.
• Adhesive skin reaction in individuals sensitive to the adhesive components.

Situations Where Opsite Film Dressing Is Appropriate

• Dry or minimally producing closed wounds, where waterproof protection and visual monitoring are the clinical objectives
• IV cannula and central line insertion site protection, where the transparent film allows the site to be observed while keeping it covered and protected from external contamination
• Post operative cover over closed suture lines where waterproof protection and wound monitoring without removal are needed
• Protection of skin at risk from friction, shear, or pressure, where a conformable transparent film barrier reduces mechanical stress on the skin surface
• Secondary film overlay over primary wound contact materials, including alginate, hydrofibre, and gel dressings, where the film seals the wound area and provides waterproof outer protection
• Adults and children where clinically appropriate

Opsite Film Dressing vs Mepore Film

Opsite Film Dressing (Smith & Nephew) and Mepore Film (Molnlycke Health Care) are both transparent waterproof polyurethane film dressings in the same clinical category with the same properties: waterproof, moisture vapour permeable, transparent, self-adhesive, no absorbency. Both are used for the same clinical situations. The choice between them is typically based on local formulary, clinician preference, or product availability rather than a clinical distinction between the two products.
