- Antidiabetic medicines: bendroflumethiazide may raise blood glucose levels, potentially requiring an adjustment to the dose of insulin or oral antidiabetic medicines.

Bendroflumethiazide is a prescription-only medicine. The clinical consultation is the essential first step. A prescription will only be issued if a registered prescriber determines, based on their individual assessment of your circumstances, that bendroflumethiazide is clinically appropriate for you.

Bendroflumethiazide tablets are used to treat hypertension (high blood pressure) and oedema (fluid retention) associated with conditions such as heart failure, kidney disease, and liver cirrhosis. They belong to the thiazide diuretic class of medicines and work by increasing urine production to remove excess fluid and lower blood pressure. It is a Prescription Only Medicine. A valid prescription from a registered prescriber, following a clinical consultation, is required before bendroflumethiazide can be supplied.
Bendroflumethiazide is still a licensed treatment for hypertension, but current NICE guideline NG136 on hypertension in adults recommends thiazide-like diuretics such as indapamide or chlortalidone in preference to older thiazides such as bendroflumethiazide, where a diuretic is indicated. This is because thiazide-like diuretics have a longer duration of action and may have a more favourable metabolic profile. Bendroflumethiazide may still be appropriate for patients already established on it or where a prescriber determines it remains clinically suitable. Your prescriber will assess this during your consultation.
The two strengths contain different amounts of the same active ingredient. Bendroflumethiazide 2.5mg is the standard dose used for hypertension. Increasing the dose above 2.5mg does not generally produce a meaningfully greater blood pressure reduction but increases the risk of electrolyte disturbances such as low potassium. Bendroflumethiazide 5mg is used primarily for oedema, where a greater diuretic effect is required. Your prescriber will determine the appropriate strength based on the condition being treated and your clinical response.
Yes. Low potassium (hypokalaemia) is one of the most important side effects of thiazide diuretics including bendroflumethiazide. Thiazides increase potassium excretion in the urine, and over time this can cause potassium levels to fall. Symptoms of low potassium include muscle cramps, weakness, tiredness, and an irregular heartbeat. Your prescriber will monitor your potassium and other electrolytes periodically during treatment. If you experience muscle cramps or weakness, contact your prescriber. Do not take potassium supplements without medical advice.
Take bendroflumethiazide in the morning. Diuretics increase the frequency of urination for several hours after each dose. Taking your tablet in the morning means this effect occurs during the day rather than disturbing your sleep. Always follow your prescriber's specific instructions regarding timing. If you have been prescribed bendroflumethiazide to take more than once daily, take the second dose no later than mid-afternoon, as directed by your prescriber.
Bendroflumethiazide is contraindicated in patients with gout or a history of gout attacks. Thiazide diuretics raise uric acid levels in the blood by reducing the excretion of uric acid through the kidneys. In patients who are susceptible to gout, this can trigger an acute attack. If you have a history of gout, inform your prescriber during your consultation. They will assess whether bendroflumethiazide is appropriate for you or whether an alternative antihypertensive medicine would be safer.
Bendroflumethiazide can be used in patients with diabetes, but with caution. Thiazide diuretics can impair glucose tolerance and raise blood glucose levels, which may make diabetes more difficult to control or unmask previously undiagnosed diabetes in susceptible individuals. If you have diabetes and take bendroflumethiazide, more frequent blood glucose monitoring may be appropriate, particularly when starting treatment or adjusting your dose. Inform your prescriber and diabetes care team that you are taking or considering bendroflumethiazide.
Yes. Periodic blood tests are an important part of safe bendroflumethiazide treatment. Thiazide diuretics can affect potassium, sodium, and uric acid levels and can also affect kidney function and blood glucose. Blood tests allow your prescriber to detect any imbalances and adjust your treatment accordingly. Monitoring is particularly important when starting treatment, after any dose change, in older patients, and in patients with kidney impairment or diabetes. Always attend any monitoring appointments or blood tests requested by your prescriber or GP.
Alcohol can lower blood pressure and may enhance the blood pressure-lowering effect of bendroflumethiazide, increasing the risk of dizziness, lightheadedness, or fainting, particularly when standing up from a seated or lying position. Alcohol can also increase the risk of dehydration when combined with a diuretic. If you choose to drink alcohol while taking bendroflumethiazide, do so in moderation and be aware of how it affects you. If you have concerns, speak to your pharmacist or prescriber.
No. Both are diuretics, but they belong to different classes and work in different parts of the kidney. Bendroflumethiazide is a thiazide diuretic that acts on the distal convoluted tubule and produces a milder, more sustained diuretic effect. Furosemide is a loop diuretic that acts on the loop of Henle and produces a more potent and rapid diuretic effect. Furosemide is generally used for more significant fluid retention and heart failure, while bendroflumethiazide is used for hypertension and milder oedema. Your prescriber will choose the most appropriate diuretic based on your clinical needs.
