How Do Olmetec Tablets Work?

How to Take Olmetec Tablets

  • Take one tablet once daily, at approximately the same time each day.
  • Olmetec can be taken with or without food.
  • Swallow the tablet whole with a glass of water.
  • If you miss a dose, take it as soon as you remember on the same day. If you only remember the following day, skip the missed dose and continue with your normal schedule. Do not take two doses on the same day.
  • Do not stop taking Olmetec without speaking to your prescriber first. Blood pressure requires ongoing management, and stopping suddenly can cause it to rise again.
  • Your prescriber will typically start you at 10 mg or 20 mg once daily and may increase the dose after two weeks if blood pressure is not adequately controlled. Blood pressure should be reviewed regularly. Kidney function and potassium levels may be checked periodically during treatment. Attend all follow up appointments so that your response to treatment can be safely assessed.

Dosage and Strengths Available

Important: Sprue-like Enteropathy

This condition causes severe, chronic diarrhoea with significant weight loss and, in some cases, serious nutritional deficiency. It can develop months or years after starting olmesartan treatment, making the connection to the medicine easy to miss.

The MHRA issued a Drug Safety Update in 2013 and the current UK SmPC reflects this risk. If you develop persistent severe diarrhoea combined with significant weight loss during olmesartan treatment, contact your prescriber promptly. Other causes will need to be excluded, but if sprue-like enteropathy related to olmesartan is confirmed, stopping the medicine typically leads to improvement. Do not stop your blood pressure medicine without first seeking medical advice.

Serious Side Effects: Seek Urgent Medical Attention

Go to your nearest accident and emergency department or call 999 immediately if you experience:

  • Angioedema: sudden swelling of the face, lips, tongue, or throat. This is rare but potentially life threatening. If swelling affects the throat and makes breathing difficult, seek emergency help immediately. If you have had angioedema from any ARB before, you must not take Olmetec.
  • Signs of severely raised potassium such as muscle weakness, an irregular heartbeat, or confusion
  • Signs of a serious reduction in kidney function such as a significant fall in urine output

Report any suspected side effect to your prescriber, pharmacist, or via the MHRA Yellow Card scheme at yellowcard.mhra.gov.uk.

Who Can Take Olmetec Tablets?

Olmetec Tablets May Be Suitable If You:

  • Are an adult aged 18 or over with a confirmed diagnosis of essential hypertension
  • Cannot tolerate ACE inhibitors due to a persistent dry cough and require an alternative antihypertensive medication in the ARB class
  • Are already prescribed Olmetec or generic olmesartan and require a repeat prescription through an online prescribing service

Olmetec Tablets Are Not Suitable If You:

  • Are pregnant or planning to become pregnant. Olmesartan is contraindicated in pregnancy. ARBs can cause serious harm to the developing baby, including kidney damage, skull abnormalities, and foetal death, particularly in the second and third trimesters. Contact your prescriber immediately if you become pregnant while taking Olmetec.
  • Are breastfeeding
  • Have severe liver disease or biliary obstruction

Important Drug Interactions

  • Potassium supplements, potassium-sparing diuretics (such as spironolactone or amiloride), and potassium-containing salt substitutes: these can raise blood potassium to dangerous levels when combined with olmesartan.
  • NSAIDs such as ibuprofen: regular use can reduce the blood pressure lowering effect of olmesartan and increase the risk of kidney problems.
  • Lithium: ARBs can increase lithium blood levels, raising the risk of toxicity. Close monitoring is required.
  • Aliskiren: contraindicated in combination with olmesartan in patients with diabetes or significant kidney impairment.
  • Colesevelam: this bile acid sequestrant can reduce the absorption of olmesartan. The SmPC advises that Olmetec should be taken at least four hours before colesevelam.

  • Aliskiren: contraindicated in combination with olmesartan in patients with diabetes or significant kidney impairment.
  • Colesevelam: this bile acid sequestrant can reduce the absorption of olmesartan. The SmPC advises that Olmetec should be taken at least four hours before colesevelam.

Olmetec tablets are used to treat essential hypertension (high blood pressure) in adults. The active ingredient is olmesartan medoxomil, an angiotensin receptor blocker (ARB) that lowers blood pressure by blocking the receptor through which the hormone angiotensin II causes blood vessels to narrow. Olmetec is taken once daily. It is a Prescription Only Medicine and requires a valid prescription from a registered prescriber following a clinical consultation.

The three strengths contain different amounts of the same active ingredient, olmesartan medoxomil. The 20 mg tablet is the standard starting dose for most adults. The 10 mg tablet is a lower starting option for patients who may be more sensitive to blood pressure lowering, including older patients or those with kidney impairment. The 40 mg tablet is the maximum dose, used when 20 mg has not provided adequate blood pressure control. All dose decisions are made by your prescriber based on your individual blood pressure response.

Olmetec, losartan, and irbesartan all belong to the ARB class and work through the same mechanism of blocking the AT1 receptor. They are different medicines with slightly different pharmacokinetic profiles. One difference to note is that olmesartan has been specifically associated with sprue-like enteropathy, a rare intestinal condition, which is not a recognised class effect of other ARBs. Your prescriber will choose the ARB most appropriate for your individual clinical circumstances, taking into account your full medical history and current medicines.

Sprue-like enteropathy is a rare intestinal condition that has been associated specifically with olmesartan. It causes severe, persistent diarrhoea and significant weight loss and can occur months or years after starting treatment. It is thought to be an immune mediated reaction. The condition is rare, and most patients taking olmesartan do not experience it. However, if you develop persistent severe diarrhoea with unexplained weight loss while taking Olmetec, you should contact your prescriber promptly so that the cause can be investigated.

No. Olmetec contains olmesartan, an ARB, which does not cause the dry persistent cough associated with ACE inhibitors such as lisinopril and ramipril. The cough caused by ACE inhibitors results from bradykinin accumulation, which does not occur with ARBs. Olmetec is often prescribed specifically for patients who have developed a troublesome cough on an ACE inhibitor. A small number of patients taking any ARB may occasionally develop a cough, but this is uncommon and less frequently reported than with the ACE inhibitor class.

No. Olmetec is contraindicated in pregnancy. Olmesartan medoxomil is an ARB, and ARBs can cause serious harm to the developing baby, particularly in the second and third trimesters, including kidney damage, reduced skull formation, and foetal death. If you are planning a pregnancy or find out you are pregnant while taking Olmetec, contact your prescriber immediately so that a safe alternative can be prescribed. Do not stop taking your blood pressure medicine suddenly without first seeking medical guidance.

Yes. Raised blood potassium levels (hyperkalaemia) are a recognised side effect of Olmetec and other ARBs. Olmesartan reduces aldosterone activity, which normally promotes potassium excretion by the kidneys. The risk of hyperkalaemia is higher in patients with kidney disease, diabetes, or those also taking potassium supplements, potassium-sparing diuretics, or other medicines that raise potassium. Your prescriber may arrange periodic blood tests to check potassium levels. Report muscle weakness, palpitations, or an irregular heartbeat to your prescriber promptly.

If you develop persistent severe diarrhoea, particularly if accompanied by unexplained weight loss, while taking Olmetec, you should contact your prescriber promptly. These symptoms could indicate sprue-like enteropathy, a rare intestinal condition that has been specifically associated with olmesartan medoxomil. Your prescriber will investigate the cause and, if olmesartan is suspected, may recommend stopping the medicine. Do not stop Olmetec without medical guidance. If you are severely unwell, seek urgent medical attention.

Olmetec is the originator brand of olmesartan medoxomil, manufactured by Daiichi Sankyo. Generic olmesartan products contain the same active ingredient at the same doses and are subject to the same regulatory standards for bioequivalence. Both the branded and generic versions are equally valid clinically. Your prescriber may prescribe either by brand name or by the generic name olmesartan.
