Candesartan Tablets May Be Suitable If You:

Candesartan Tablets Are Not Suitable If You:

- Signs of significant kidney deterioration such as a marked reduction in urine output or significant swelling.
- Symptoms of very high potassium levels such as muscle weakness, an irregular heartbeat, or tingling and numbness.
- Signs of a serious allergic reaction such as difficulty breathing or a severe skin reaction.

Candesartan Tablets May Be Suitable If You:
- Are an adult with hypertension who requires antihypertensive treatment, including as an alternative to an ACE inhibitor if you have developed an ACE inhibitor-related cough.
- Have heart failure with reduced left ventricular systolic function and cannot tolerate an ACE inhibitor or are receiving candesartan as an addition to ACE inhibitor therapy under specialist supervision.
- Are already prescribed candesartan and require a repeat prescription through an online prescribing service.
Candesartan Tablets Are Not Suitable If You:
- Are pregnant or planning to become pregnant - candesartan must not be used during pregnancy.
- Have severe liver impairment or cholestasis.
- Are taking aliskiren alongside candesartan if you have diabetes or kidney impairment (eGFR below 60 ml/min/1.73m2).
- Have a known allergy or hypersensitivity to candesartan cilexetil or any excipient listed in the SmPC.
- Are under 18 years of age (this service is for adults only).
Cautions
Particular caution is required in patients with kidney impairment, as candesartan can affect kidney function, particularly in patients with renal artery stenosis or those taking NSAIDs. Patients with a history of ACE inhibitor-induced angioedema should inform their prescriber, as angioedema is possible with ARBs too, though less common. Caution is required in patients who are volume-depleted (such as those taking high-dose diuretics), as the first dose may cause a significant fall in blood pressure. Kidney function and potassium levels should be monitored at the start of treatment and periodically thereafter.
Important Drug Interactions
Candesartan can interact with a number of commonly prescribed medicines. Tell your prescriber about all medicines you take, including over-the-counter products and herbal supplements. The following interactions are of particular clinical importance:
- Potassium-sparing diuretics (such as spironolactone, amiloride), potassium supplements, or salt substitutes containing potassium: combining these with candesartan significantly increases the risk of raised blood potassium (hyperkalaemia).
- ACE inhibitors: combining candesartan with an ACE inhibitor increases the risk of hypotension, hyperkalaemia, and kidney impairment. This combination should only be used under specialist supervision in heart failure patients.
- Aliskiren: the combination of candesartan with aliskiren is contraindicated in patients with diabetes or kidney impairment and is not recommended in other patients.
- NSAIDs such as ibuprofen: these can reduce the antihypertensive effect of candesartan and increase the risk of kidney problems, including acute kidney injury.
- Lithium: candesartan can increase lithium levels, raising the risk of lithium toxicity. Close monitoring is required.
- Other antihypertensive medicines: the blood pressure-lowering effect of candesartan may be enhanced when combined with other antihypertensive agents.

Candesartan is a prescription-only medicine. The clinical consultation is the essential first step. A prescription will only be issued if a registered prescriber determines, based on their individual assessment of your circumstances, that candesartan is clinically appropriate for you.

Candesartan tablets are used to treat hypertension (high blood pressure) in adults and to manage heart failure with reduced left ventricular systolic function in adults who cannot tolerate ACE inhibitors. Candesartan belongs to the angiotensin receptor blocker (ARB) class of medicines. It works by blocking the action of angiotensin II on blood vessels, causing them to relax and reducing blood pressure. It is a prescription-only medicine. A valid prescription from a registered prescriber, following a clinical consultation, is required before candesartan can be supplied.
Both candesartan and ACE inhibitors act on the renin-angiotensin-aldosterone system to lower blood pressure but by different mechanisms. ACE inhibitors prevent angiotensin II from being formed. Candesartan blocks the receptor at which angiotensin II acts. A key practical difference is that ACE inhibitors frequently cause a persistent dry cough as a side effect, while candesartan is much less likely to do so. For this reason, candesartan is often prescribed for patients who have developed a cough on an ACE inhibitor. Both classes carry similar risks of hyperkalaemia, hypotension, and kidney impairment.
No. Candesartan must not be used during pregnancy. ARBs can cause serious harm to the developing foetus, including kidney damage, low amniotic fluid, skull abnormalities, and, in severe cases, death of the unborn baby. This risk is particularly high during the second and third trimesters. If you become pregnant while taking candesartan, stop taking it and contact your prescriber immediately. Women of childbearing potential should use effective contraception during candesartan treatment. Your prescriber will advise on suitable alternatives for blood pressure management during pregnancy.
Candesartan is much less likely to cause a persistent dry cough than ACE inhibitors. The dry cough associated with ACE inhibitors is caused by a build-up of bradykinin, which does not occur with ARBs such as candesartan. This is one of the main reasons patients are switched from an ACE inhibitor to candesartan. A small number of patients taking candesartan may report upper respiratory symptoms, but a persistent dry cough of the type caused by ACE inhibitors is not a recognised class effect of ARBs.
The three most commonly used strengths contain different amounts of the same active ingredient. Candesartan 8mg is the standard starting dose for most adults with hypertension. If blood pressure is not adequately controlled, the dose may be increased to 16mg, and then to 32mg (the maximum dose) if needed. For heart failure, the target dose is 32mg once daily, reached through gradual titration starting from 4mg or 2mg. All dose increases are made by your prescriber based on your response and tolerability. Do not change your dose without medical advice.
Yes. Candesartan can raise blood potassium levels (hyperkalaemia) by reducing the effects of aldosterone, which normally promotes potassium excretion. The risk is higher in patients with kidney impairment, diabetes, heart failure, or those taking other medicines that raise potassium, such as potassium-sparing diuretics, potassium supplements, or other drugs that block the renin-angiotensin-aldosterone system. Your prescriber will monitor your potassium and kidney function at the start of treatment and periodically during treatment. Symptoms of high potassium include muscle weakness, irregular heartbeat, and tingling.
You should avoid regular use of ibuprofen and other non-steroidal anti-inflammatory drugs (NSAIDs) while taking candesartan. NSAIDs can reduce the blood pressure lowering effect of candesartan and, when used together regularly, can significantly increase the risk of kidney problems, including acute kidney injury. If you need pain relief while taking candesartan, paracetamol is generally a safer alternative. Always check with your pharmacist before combining medicines, and inform your prescriber of all medicines you take.
Candesartan begins to lower blood pressure within a few hours of the first dose. The full antihypertensive effect typically develops within four weeks of starting treatment at the effective dose. For heart failure, the clinical benefits, including improvement in symptoms and reduction in hospitalisation risk, develop more gradually over several months of treatment at the target dose. Your prescriber may arrange a blood pressure review after four to eight weeks to assess your response and determine whether any dose adjustment is needed.
Yes. Your prescriber will typically arrange blood tests to monitor your kidney function and potassium levels before and during treatment with candesartan. This is particularly important at the start of treatment, after any dose change, and in patients with kidney impairment, heart failure, or diabetes. Monitoring helps ensure that kidney function and potassium levels remain within a safe range. Always attend any follow-up appointments or blood tests requested by your prescriber or GP.
No, though both are ARBs in the same drug class. Candesartan and losartan both block the AT1 angiotensin II receptor and have similar indications. Differences include their duration of action, available strengths, dosing schedules, and individual patient response. Candesartan has a longer duration of action and a slightly different binding profile to the receptor compared to losartan. Both are used for hypertension. Candesartan is also specifically licensed for heart failure where ACE inhibitors are not tolerated. Your prescriber will choose the most appropriate ARB for your clinical situation.
