How to Take Perindopril Tablets

- Take Perindopril once daily, in the morning before a meal. The SmPC for Perindopril erbumine specifically recommends taking the tablet before eating, as food reduces the conversion of Perindopril to the active perindoprilat.
- Swallow the tablet whole with a glass of water.
- If you miss a dose, take it as soon as you remember on the same day. If you only remember the following day, skip the missed dose and continue with your normal schedule. Do not take two doses on the same day.
- Do not stop taking Perindopril without speaking to your prescriber first. Stopping treatment can cause blood pressure to rise again or worsen heart failure symptoms.
- Your prescriber will typically start you at a low dose and increase it over several weeks based on your blood pressure response, kidney function, and potassium levels. Who Can Take Perindopril Tablets?
Perindopril Tablets May Be Suitable If You:

Perindopril is a Prescription Only Medicine. The clinical consultation is the essential first step. A prescription will only be issued if a registered prescriber concludes, based on their individual assessment of your circumstances, that Perindopril is clinically appropriate for you.

Perindopril tablets are used in adults for three licensed purposes: the treatment of hypertension (high blood pressure), symptomatic heart failure alongside conventional therapy, and reduction of cardiovascular risk in patients with stable coronary artery disease. Perindopril is an ACE inhibitor that acts on the renin-angiotensin system to lower blood pressure and protect cardiovascular health. It is a Prescription Only Medicine requiring a valid prescription from a registered prescriber following a clinical consultation.
Both are forms of Perindopril but they contain different salt components. Perindopril erbumine comes in 2 mg, 4 mg, and 8 mg strengths. Perindopril arginine comes in 2.5 mg, 5 mg, and 10 mg strengths. These formulations are not interchangeable at the same numerical dose because the salt component forms part of the total tablet weight. A Perindopril erbumine 4 mg tablet is clinically equivalent to a Perindopril arginine 5 mg tablet. If you switch between brands or suppliers, always check with your pharmacist that you are receiving the correct equivalent dose.
The SmPC for Perindopril erbumine recommends taking the tablet in the morning before a meal. Food reduces the absorption of Perindopril and, more importantly, reduces the conversion of Perindopril to its active form, perindoprilat, in the body. Taking Perindopril on an empty stomach helps to ensure you receive the full therapeutic effect. Take it first thing in the morning before breakfast and at approximately the same time each day. If your prescriber has given you specific advice about timing, follow their instructions.
Yes. A dry persistent cough is the most commonly reported side effect of Perindopril and is a class effect of all ACE inhibitors. It occurs because ACE inhibitors reduce the breakdown of bradykinin, a compound that can irritate the airways. The cough can develop at any point during treatment. It is not harmful but can be troublesome. If the cough significantly affects your quality of life, speak to your prescriber. Switching to an angiotensin receptor blocker (ARB) such as losartan or irbesartan, which does not cause bradykinin accumulation, is a common alternative.
No. Perindopril is contraindicated in pregnancy. ACE inhibitors can cause serious harm to the developing baby, including kidney damage, reduced skull development, and foetal death, particularly when taken during the second and third trimesters. If you are planning a pregnancy or find out you are pregnant while taking perindopril, contact your prescriber immediately so that a safe alternative can be prescribed. Do not stop taking your blood pressure or heart medicine suddenly without first seeking medical guidance.
Yes. Perindopril has a licensed indication for reducing the risk of cardiovascular events in patients with stable coronary artery disease, based on evidence from the EUROPA clinical trial. In this indication, Perindopril may be prescribed for patients who have had a heart attack or have established coronary artery disease, even if their blood pressure is not particularly elevated. It is also used in symptomatic heart failure. Your prescriber will determine which indication applies to your situation and will prescribe the appropriate dose accordingly.
You should avoid regular use of ibuprofen and other non-steroidal anti-inflammatory drugs (NSAIDs) while taking perindopril. NSAIDs can reduce the blood pressure lowering effect of Perindopril and, when used together over time, can increase the risk of kidney problems. This is particularly relevant in older patients and those with reduced kidney function. Paracetamol is generally a more appropriate choice for mild to moderate pain in patients taking ACE inhibitors. Always check with your pharmacist before combining medicines.
Perindopril and ramipril are both ACE inhibitors with similar mechanisms of action. Key differences include their licensed indications, dosing schedules, and pharmacokinetic profiles. Ramipril has a broader range of licensed UK indications including post myocardial infarction heart failure and nephropathy prevention. Perindopril has a specific licensed indication for stable coronary artery disease risk reduction (EUROPA trial evidence). Both are taken once daily but at different times: Perindopril erbumine is taken before a meal, whereas ramipril can be taken with or without food. Your prescriber will choose the most appropriate ACE inhibitor for your individual circumstances.
For most patients with hypertension, heart failure, or coronary artery disease, Perindopril is a long-term medicine. These are ongoing conditions that require continuous management. Stopping Perindopril without medical advice can allow blood pressure to rise again, worsen heart failure, or remove the cardiovascular protection it provides. Your prescriber will review your treatment periodically to assess whether Perindopril remains appropriate. Do not stop taking it without first speaking to your prescriber.
