Take one tablet every day at the same time for 21 consecutive days. After your 21st tablet, stop for exactly seven days. A withdrawal bleed will usually occur during this week. Start your next 21-tablet pack on day 29, the day after the seven-day break ends, regardless of whether any bleeding has stopped. The break must not exceed seven days.

  • Sudden severe headache, visual disturbance, weakness in the face, arm, or leg, or difficulty speaking. These may indicate a stroke.
  • Severe abdominal pain, which may indicate a liver problem or ectopic pregnancy.
  • Jaundice: yellowing of the skin or whites of the eyes.

Blood Clot Risk and Gestodene

All combined oral contraceptives carry a small increased risk of venous thromboembolism compared with women not using hormonal contraception. Gestodene-containing pills including Femodette carry a somewhat higher VTE risk than levonorgestrel-containing pills, and a broadly similar risk to pills containing desogestrel or drospirenone. The lower oestrogen dose in Femodette does not eliminate this risk. Your prescriber will assess your individual VTE risk factors before prescribing.

Important Drug Interactions

Enzyme-inducing medicines can reduce the effectiveness of Femodette by increasing the rate at which the active hormones are metabolised. These include:

  • Rifampicin and rifabutin
  • Certain antiepileptic medicines including phenytoin, carbamazepine, phenobarbital, and topiramate
  • Some HIV medicines including ritonavir and efavirenz
  • St John's Wort, a herbal supplement available without prescription

Always disclose your full medication list, including herbal remedies and supplements, at every consultation. If you are prescribed an enzyme-inducing medicine whilst taking Femodette, additional contraceptive precautions or an alternative method may be required.

Femodette Is Not Suitable If You:

  • Have or have ever had a blood clot (deep vein thrombosis, pulmonary embolism, or arterial thrombosis)
  • Have or have ever had a stroke or heart attack
  • Have migraine with aura
  • Have uncontrolled high blood pressure
  • Have liver disease or liver tumours
  • Have or have ever had breast cancer or another oestrogen-sensitive cancer
  • Are pregnant or think you may be pregnant
  • Are breastfeeding a baby under six weeks old
  • Are a smoker aged 35 or over
  • Have diabetes with vascular complications

This Medicine May Be Appropriate If You:

  • Are a healthy adult woman seeking reliable daily hormonal contraception
  • Have previously experienced oestrogen-related side effects such as nausea, headache, or breast tenderness on a 30 or 35 microgram combined pill
  • Have no personal or family history of blood clots, stroke, or cardiovascular disease
  • Do not have migraine with aura and are not a heavy smoker
  • Are not currently taking medicines that interact significantly with combined oral contraceptives

Use With Caution If You:

  • Are a smoker under the age of 35. Smoking at any age increases cardiovascular risk with oestrogen-containing contraceptives.
  • Have a BMI above 30. Obesity is an independent risk factor for VTE.
  • Have a family history of blood clots in a first degree relative under the age of 50.
  • Have a history of depression or are currently experiencing low mood. Hormonal contraceptives can affect mood in some women.
  • Are planning long-haul travel or extended periods of immobility. These independently increase VTE risk.
  • Have previously had high blood pressure in pregnancy or other oestrogen-sensitive conditions.

Femodette contains ethinylestradiol 20 micrograms, while Femodene contains 30 micrograms. Both use a standard 21-day active tablet, seven-day break schedule, and both are manufactured by Bayer plc. The lower oestrogen dose in Femodette may suit women who experienced oestrogen-related side effects such as nausea or breast tenderness on Femodene or another higher-dose combined pill. The contraceptive mechanism and gestodene-related VTE risk class are the same for both.

Yes. Femodette contains ethinylestradiol 20 micrograms, which is at the lower end of the oestrogen dose range used in combined oral contraceptives in the UK. Many other combined pills contain 30 or 35 micrograms. A lower oestrogen dose is sometimes associated with reduced oestrogen-related side effects such as nausea, headache, and breast tenderness, particularly in the early weeks of use. However, this is not guaranteed, as individual responses vary. A lower dose does not mean lower contraceptive effectiveness when the pill is taken correctly and consistently.

When taken correctly and consistently, Femodette is over 99% effective at preventing pregnancy. This level of effectiveness applies to all combined oral contraceptives taken as directed. In typical use, which accounts for occasional missed or late tablets, effectiveness is lower. Taking one tablet at the same time every day, following the missed pill guidance carefully, and restarting new packs promptly after the seven-day break all contribute to maintaining consistent protection. If you are ill with vomiting or diarrhoea, follow the missed pill guidance in the Patient Information Leaflet.

If you are fewer than 24 hours late, take the missed tablet as soon as you remember and continue as normal. You remain protected. If you are more than 24 hours late, take the missed tablet immediately and take the next at your usual time, even if that means two in one day. Use additional contraception for seven days. If you miss two or more consecutive tablets, take the most recently missed one immediately, leave the others, continue the pack, and use additional contraception for seven days. If tablets were missed in week one and you had unprotected sex, contact your prescriber or pharmacist about emergency contraception.

Many women taking combined oral contraceptives experience lighter and less painful withdrawal bleeds. This is a recognised secondary effect of combined hormonal contraception and is related to the suppression of the natural hormonal cycle. Femodette is licensed as a contraceptive, not specifically as a treatment for dysmenorrhoea or heavy periods. However, if lighter periods are also a benefit you experience during use, this is consistent with the known effects of this type of pill. Any decisions about using it specifically for period management should be discussed with your prescriber.

If you experience migraine with aura, Femodette is contraindicated. All oestrogen-containing combined pills are associated with an increased risk of stroke in women who experience aura, and this applies to Femodette regardless of its lower oestrogen dose. This is an absolute contraindication. If you experience migraine without aura, your prescriber will conduct an individual assessment. Always disclose your full migraine history during your consultation, including whether you experience visual disturbances, sensory changes, or speech difficulties before or during headaches.

It depends on the nature and cause of the side effects you experienced. If your side effects were primarily oestrogen-related, such as persistent nausea, breast tenderness, or headaches, then switching to a lower-dose pill such as Femodette may be worth discussing with your prescriber. However, if your side effects were related to the progestogen component, the same progestogen (gestodene) is used in both Femodette and Femodene, so a switch between these two may not resolve progestogen-related symptoms. Your prescriber can advise on whether Femodette or a different pill is most appropriate based on a full review of your history.

Yes, it is possible to delay a withdrawal bleed by running two packs together: begin your next 21-tablet pack the day after finishing your current one, skipping the seven-day break. A withdrawal bleed will then occur at the end of the second pack. There is a higher chance of irregular spotting when running packs together. This approach is generally not harmful for most healthy women but is worth discussing with your prescriber before you try it, particularly if you are new to Femodette. Do not run more than two packs together without clinical guidance.

Fertility typically returns quickly after stopping Femodette. Most women ovulate within one to three months of stopping the combined pill. There is no clinical evidence that taking a combined oral contraceptive for a prolonged period reduces long-term fertility. Some women conceive within their first natural cycle after stopping, while others take a few months for a regular cycle to re-establish. If you are planning a pregnancy, allowing your natural cycle to return before conceiving makes it easier to date the pregnancy accurately. If your periods have not returned within three months of stopping, speak to your GP.
